[A6] hanging notes bug

Firstly, I must state this is a random problem so I can't specifically say
what causes it or how to guarantee reproducing it.

However, the way I usually can manage to reproduce it is to play a program
which has a sequencer, hold down a note or a few notes, press the sustain
pedal and add a few more notes, release and activate sustain again a few
times (holding for a few seconds) while randomly adding and releasing more
notes. Every so often you will need to release all notes and sustain pedal
to see if a sequence has "stuck".
Now I don't usually play the Andromeda like this, however it is a way I can
demonstrate the problem. Generally within a few minutes the problem should
occur. The A6 clock can be set to internal or external.
