[A6] OS 1.40.12 for Windows

A few questions:

Which OS contained the fix for faster envelopes?
Which OS is the latest official release, past Beta stage?  What's shiping
with new A6s?
Does upgrading the OS erase all custom patches?  I plan to back them up just
in case, but loading them back in is one less step and would be nice.  Yeah,
I'm lazy...
Will future OS updates require previous updates or can you install over the
originally released old OS?
